Can my employer retroactively charge me for insurance premuims when they forgot to charge me for 3 months?

if you are in America and do not have a written contract covering this point, yes.
of course, you can always quit before they do.
Yes they can-you were covered by the insurance so it is your responsibility to make sure it is paid. It ***** but it is legal and pretty common.

Yes legally they can. The reason for this is that your employer is the policy holder and you have a certificate of coverage under the policy, and the employer has been paying the premiums even though they forgot to withdraw it from your check. So you have had coverage for the full time.
